Client Care Liaison (JR25010): The Client Care Liaison will manage communications and scheduling with Care2Prevent clients. They will help develop systems to manage appointments and appointment communications, working with the interdisciplinary team to support client medical engagement. The Client Care Liaison will prepare reports and summaries, triage phone calls and assist in managing the daily operation of the office.
Linkage to Care Specialist (JR24421): This is an HIV/STI testing and linkage to care position. The Linkage to Care Specialist will focus on LGBTQ+ sexual health and community outreach services. These include HIV and STI testing, including phlebotomy, linkage to care for treatment and PrEP services, and STI treatment notification and linkage to care. Accurate documentation is required for all client encounters. Community outreach on the mobile unit and with organizations is required, as well as occasional nights and weekends. Must be willing to be certified in phlebotomy within 30 days of hire. This is a full-time position and is in-person with the option for occasional remote work after probationary period.
