Manager of Care Coordination (JR 18140): The Manager of Care Coordination leads the Ryan White medical and non-medical case management teams. The manager ensures grant scopes are being met and is responsible for regular reporting as well as assisting in policy development, program planning and training. Direct service responsibilities include conducting assessments of client needs, implementing service plans and identifying barriers to treatment adherence. The manager will serve as an active member of the healthcare team and will be required to work some evening clinic nights.

Temporary Medical Case Coordinator (JR 17673): The temporary Medical Case Coordinator (MCC) will carry a caseload of 35-40 clients living with HIV, addressing barriers to care and other psychosocial needs. The MCC will collaborate with the medical, outreach and behavioral health teams to connect clients to services and will also address housing, insurance and other needs. The MCC conducts initial assessments/reassessments of patient needs, implements service plans, identifies gaps in services, and provides ongoing support. MCC must complete required training within 90 days of hire and is responsible for documenting all client encounters. Evening clinic hours will be required.

Medical Case Coordinator (JR 17877): The full-time, permanent Medical Case Coordinator (MCC) will carry a caseload of 35-40 clients living with HIV, addressing barriers to care and other psychosocial needs.
